Every contestant needs a signed waiver on file with Sundance Rodeo Club before competing. Some contestants also need a W-9 on file, depending on their winnings. This page has everything you need for both.
Every contestant must have a signed general release of liability waiver on file before competing. One waiver covers a contestant for as long as they compete with SRC, with one exception: when a contestant turns 18, they need to sign a new waiver as an adult, even if a parent signed one for them before.

A completed W-9 is only required for contestants who win $2,000 or more in payouts during the year. If that's your contestant, congratulations, and we'll need the form on file before we can pay out. Download it, fill it out, and get it back to us one of these ways:

3. Hand it in at the rodeo office on rodeo day
